Information we collect
When you submit a coverage form, we collect your plan and insurer information, contact information, address, employer name when provided, and limited campaign or referral information included with the page visit. The form does not request medical records, Social Security numbers, payment-card information, or insurance card images.
When submission-attempt tracking is enabled and the database is available, we also record the date and time, the form page used, the requested page, the referring website and path when supplied by the browser, the IP address, browser or device information, marketing-attribution identifiers when present, and whether the submission was accepted or rejected. Referring-page query strings and fragments are not retained.
Cookies
This website uses a temporary, first-party session cookie to protect forms against unauthorized submissions, apply a brief submission cooldown, and display submission confirmations. The application does not use this cookie for advertising.
How we use the information
We use the submitted information to review potential insurance benefits, compare prior outcomes involving policies associated with the same employer, identify a participating office, understand which campaign page generated a request, and contact you about the request. We use technical submission records to attribute inquiries, measure form performance, prevent spam and abuse, troubleshoot errors, and protect the website. We do not use these technical records for cross-site advertising. Prior outcomes do not guarantee the result for your plan.
How information may be shared
Information may be shared with participating providers, affiliated organizations, and service providers only as needed to respond to your request, operate and protect the program, or comply with law. When email notifications are enabled, a copy of the submitted form information is sent to authorized staff through the organization’s configured email service provider. Information is not sold.
Data security and retention
Reasonable administrative and technical safeguards should be maintained on the hosting account, database, and email system. Access should be limited to authorized personnel. Form records, technical submission logs, and notification emails should be retained only as long as needed for the stated purpose, security investigations, and applicable legal obligations, then deleted or de-identified under the organization’s approved retention schedule.
